Treatment options in Kienböck's disease.

چکیده
Kienböck’s disease or lunatomalacia is an aseptic osteonecrosis of the lunate bone initiating a progressive collapse of the lunate bone, followed by total carpal collapse. The first description was made one hundred years ago (107). Thus far, its exact cause and natural history remain unknown (44). Several extrinsic and intrinsic factors have been investigated as to their role in the aetiology of the bone necrosis. Kienböck’s disease is a slowly progressive disorder with well recognised stages leading to various treatment options. However, little is known about their effectiveness, mainly because Kienböck’s disease is not very common and a long follow-up is needed to evaluate the ultimate outcome (1,84).

The goal of this cross-sectional study was to determine the prevalence of Kienböck's disease in middle-aged and elderly Japanese women and to clarify the relationship between Kienböck's disease and negative ulnar variance. T...
متن کاملKuhlmann vascularized bone grafting for treatment of Kienböck's disease: a case report
Treatment of Kienböck's disease has historically been determined by staging, ulnar variance, and presence or absence of arthritic changes. With the advent of newer techniques of vascularized bone grafting, the status of the cartilage shell of the lunate has become another factor that can influence the procedure performed. The purpose of this article is to describe the technique of Kuhlmann vasc...
متن کاملRotatory subluxation of the scaphoid in Kienböck's disease is not a cause of scapholunate advanced collapse (SLAC) in the wrist.
We have examined whether the rotatory subluxation of the scaphoid which is seen in patients with advanced Kienböck's disease is associated with scapholunate advanced collapse (SLAC) wrist. We studied 16 patients (11 men, 5 women) who had stage-IV Kienböck's disease with chronic subluxation of the scaphoid. All had received conservative treatment. The mean period of affection with Kienböck's dis...
متن کاملA comparative study of the surgical procedures to treat advanced Kienböck's disease.
We have treated a total of 16 cases of advanced Kienböck's disease, stage III and IV by Lichtman's classification, with triscaphe fusion, tendon ball replacement arthroplasty after excision of lunate, proximal row carpectomy as a salvage procedure and limited wrist fusion, since 1985. All cases were followed for minimal 16 months after each operation. Tendon ball replacement arthroplasty after ...
